Calder & Ryburn Primary Care Network is a group of four GP Surgeries collaboratively working together to provide a greater provision of proactive and personalised care within the local community.
Our Mental Health Practitioner (MHP) creates a comfortable environment, providing reassurance and active listening. The focus is on empowering patients to manage their mental health, while assessing risk throughout the session.
Our Pharmacy Team strive on a person-centered approach to ensure that our patients get the right choice of medicines at the right time. By conducting Structured Medication Reviews and working closely with other services, the aim is to improve the safety of our patients and optimise medications by improving health outcomes.
Our First Contact Physiotherapists (FCP) offer musculoskeletal appointments for patients which provides immediate access to physiotherapy. Our FCP’s are professionally qualified to treat patients without a referral from a GP or other healthcare professional.
Our Personalised Care Team (PCT) take pride in delivering a personalised care approach. It is based on shared decision-making which means patients have choice and control over the way their care is planned and delivered.
